Every three years, the Center for Opinion Research at Franklin & Marshall College performs a comprehensive community health assessment on behalf of Healthy York County Coalition, Healthy Adams County Coalition and key community partners. This three-part study includes a telephone survey of hundreds of community residents to identify their current health status and behaviors that may impact their health. Additionally, the coalitions analyze regional health-related data and conduct in-person interviews with community residents and key stakeholders.

As member of Live Well Lancaster, a coalition comprised of more than 25 mostly non-profit health & human service organizations, WellSpan Ephrata Community Hospital is an active participant in the county-wide community needs assessment process. The partnership meets regularly to discuss Lancaster County's most pressing health issues and sponsors a new health risk assessment assessment every three years.
A sub-group of Live Well Lancaster plans the annual Lancaster Health Summit. The health summit is a community forum to educate, share ideas, communicate data and results to key constituents. The summit is a collaborative effort with the Lancaster County Business Group on Health and draws more than 300 participants.
Actively participating in these groups and other similar coalitions and collaborative efforts
within the county has afforded WellSpan Ephrata Community Hospital greater insight into the community’s health status. This engagement has been beneficial as the hospital moved forward in drafting this implementation plan.

As you progress through the scheduling process, you will be able to see the offices that are accepting new patients in relation to your zip code. If you are not enrolled in MyWellSpan, go to https://my.wellspan.org, call 1-866-638-1842 or speak with a member of the staff at a participating facility to sign up. If you already have a relationship with a WellSpan practice, simply log into your account, and go to the Appointment Center section. As you progress through the scheduling process, you will be able to schedule an appointment with any provider or practice that already counts you as a patient.
