Heart failure, also known as congestive heart failure (CHF), means your heart can't pump enough blood to meet your body's needs. While CHF is a serious condition that has no cure, patients that prevent or control risk factors and aggressively manage any underlying conditions can live a full and enjoyable life. York Hospital offers a comprehensive multidisciplinary program that focuses on helping patients to manage the disease and improve quality of life.

The following services are available through the Congestive Heart Failure Program:

  • Complete physical assessment at each visit
  • Medication treatment plan with ongoing adjustments
  • Ongoing review of patient compliance
  • Frequent primary care provider updates
  • Ongoing patient education regarding heart failure including diet, physical rehab, medication, community resources, quality of life and end of life issues
  • Telemanagement to monitor weight, blood pressure and decompensation
  • Cardiologist consult for each patient and follow-up as needed


A physician referral from your primary care physician, cardiologist or other licensed provider is required for the Congestive Heart Failure Program.

About the provider search

This search will provide you with WellSpan Medical Group and Northern Lancaster County (Ephrata) Medical Group primary care physicians and specialists. If we don’t have a WellSpan Medical Group physician to meet your criteria, the search will expand to include community physicians who partner with WellSpan Medical Group physicians through the WellSpan Provider Network or provide care to patients on the Medical Staffs of WellSpan’s Hospitals.
