The Transitional Care Unit (TCU) is a 19-bed skilled nursing unit which offers transitional care between the hospital and home. We offer medical supervision and care to patients who are too well to stay in the hospital but are not ready to return home.
The TCU is designed for patients whose goal is to return to their home in a short period of time. More than 80 percent of patients discharged from the TCU return to their former level of living.
This facility features seven semi-private rooms and five private rooms.
The average length of stay is 10 to 20 days where the patient may be provided with physical, occupational and speech therapy services up to six days per week. These patients typically do not qualify for, or cannot tolerate the three hours of therapy required for inpatient rehabilitation. Admission to TCU may require a minimum three day inpatient hospital stay depending on insurance guidelines.
While the TCU is primarily a Medicare unit, other forms of insurance may cover the care. Medicare and/or insurance coverage determination is made on or prior to admission by assessing the patient’s current orders, needs, and physical condition. The patient must meet Medicare and /or any other health insurance criteria for skilled care in order to be eligible for coverage. Patients must require the services of a licensed nurse or licensed therapist for some aspect of care. Custodial care and intermediate care are not covered services.

If we don’t have a WellSpan Medical Group physician to meet your criteria, the search will expand to include community physicians who partner with WellSpan Medical Group physicians through the WellSpan Provider Network or provide care to patients on the Medical Staffs of WellSpan’s Hospitals.×

If you are not enrolled in MyWellSpan, go to https://my.wellspan.org, call 1-866-638-1842 or speak with a member of the staff at a participating facility to sign up. If you already have a relationship with a WellSpan practice, simply log into your account, and go to the Appointment Center section. As you progress through the scheduling process, you will be able to schedule an appointment with any provider or practice that already counts you as a patient.
