It's quick and easy to use the MyWellSpan portal to access and view your health information — any time, anywhere. To request a copy of your medical record, follow these steps:
- Under the menu, go to Sharing > Request Medical Records.
- Provide the requested information.
Alternate method: online request
To request your medical record electronically and have the request sent automatically to be processed, use the link below. Remember to have your driver's license or other photo ID ready to confirm your identity.
Please note: You will be redirected to a third-party system managed by MRO. WellSpan works with the release of information vendor, MRO, to coordinate providing copies of medical records to patients and authorized representatives.

Alternate method: paper form
If you’re not able to use MyWellSpan, you can print and complete the Authorization to Use or Disclose Health Information form, available in English, Español, Nepali, and Haitian Creole (PDF).
Follow the instructions at the bottom of the Authorization to Use or Disclose Health Information form to mail, email, or fax your completed form.
Request a change to your medical record

With a few clicks in the MyWellSpan portal, you can instantly submit a request to change your medical record:
- Under the menu, go to the Document Center.
- Click on Requested Records & Record Amendments.
- Select Complete a Request for Amendment Form.
- Provide as much information as possible. This includes the type of documentation (progress note, discharge summary, problem list, etc.) and the date of the documentation.
Alternate method: online or paper form
If you're not able to use MyWellSpan, you can complete the Patient Requested Amendment Form (English - PDF) or Español (PDF) to print and mail to the address on the form. You can also complete an email version of the Request for Record Amendment form.
Frequently Asked Questions
Who may request medical records?
To request medical records, you must be the patient, or parent, or legal guardian (proof of guardianship is required) of the patient for whom you seek records.
How do I obtain medical records of my deceased loved ones?
The executor or administrator of the decedent’s estate may have access to all medical records. In absence of an executor or administrator, the next of kin responsible for the disposition of the remains may have access to medical records excluding mental health. Paperwork to support requester’s relationship to the decedent needs to submitted with the Authorization. Use the Next of Kin Verification Form to request medical records for a deceased loved one.
Are there fees associated with requesting medical records?
Records delivered via MyWellSpan – no charge
Records sent directly to another healthcare provider – no charge
Records delivered to the patient via email or fax - $6.50 plus tax
Records delivered to the patient via mail - $6.50 plus shipping, handling, and tax
Records delivered to insurance company, attorney or other third party. The fee allowable by applicable state.
How long will it take to process my request?
Average turnaround time is 15 calendar days, however it may take up to 30 days to ship.
How do I request records from multiple locations, i.e. hospitals and/or medical groups?
