How it works
A WellSpan nurse navigator helps connect you (or covered family members over the age of 18) with a primary care provider and schedules your first virtual appointment. After the initial nurse navigator visit, you can get:
- Unlimited access to a WellSpan primary care provider, including after hours, weekends and holidays
- Care coordination for chronic conditions, such as diabetes or asthma, and common behavioral health conditions
- Preventive screenings including mammography, at-home colon cancer screening and cardiac calcium scoring
- Wellness coaching, a wellness portal and annual at-home biometric screening
All at $0 copay or deductible for your primary care appointments and included screening procedures.
Services are currently available only in Pennsylvania (PA) and Maryland (MD) due to state-specific licensing regulations.
