We’re excited to let you know that you’ve been selected as an unpaid intern. Before you start your internship, you’ll need to fill out and submit a few required items.

Avoid delays by completing all the necessary forms and providing all requested documentation.
Step 1
Review the internship checklist for instructions and requirements. Complete and submit the following within one week of receiving the welcome email from WellSpan Health:
- Complete the intern data sheet. WellSpan Health will be notified electronically when you’ve completed this form.
- Email a headshot photo (in JPEG or PNG format) of yourself for your internship badge. The photo should be taken within six weeks of your start date, and should be:
- a forward-facing headshot of you in professional work attire, similar to your passport or driver's license photo with your entire face visible and not obstructed by sunglasses, hats or other items.
- in focus (no filters) and of you alone with no one else in the frame. Don’t use a selfie or a photo of multiple people with the other people cropped out of the picture.
- positioned in front of a solid light-colored plain background or surface with good natural lighting. Make sure there are no shadows, glares or dark spots in the picture.
Step 2
Submit the following at least three weeks before the anticipated start date of your internship. Note: These items must be emailed to WellSpan at internships@wellspan.org in PDF format. Don’t submit photos or screenshots, and don’t save the files to an online location (such as Google Drive).
- acknowledgement of unpaid intern handbook
- code of conduct
- online test: Please watch for further email communication from WellSpan regarding “Non-Employee WellSpan Access Information” for creation of a username and password. Completion of an online orientation will then be assigned. Please complete the training and test no greater than 30 days after receiving the email communication. Failure to do so will result in deactivation of your account.
- vaccination documentation form and all medical documentation required as listed on this form.
- consent form and the three required background reports and clearances. Copies will be accepted, but originals will be required on your first day at your internship location.
- vehicle parking information form
- copy of photo identification (for example, driver’s license, passport, etc.)

Step 3
After you complete steps 1 and 2, we’ll email your host department supervisor confirming that you’re cleared to begin your internship. You’ll be copied on the email.
