If you've had a visit or hospital stay with us recently, you may have been asked about housing, transportation or other everyday life factors. That's because we aim to screen every patient annually to understand basic barriers that may be preventing them from achieving good health.
WellSpan strives to not only take care of people when they are ill but to help them stay healthy. This approach means addressing health factors beyond the walls of our facilities. This focus on your daily life connects to a broader concept: the social determinants of health. These are the conditions in which you are born, live, learn, work, play, worship and age that impact health, function and quality of life. Improving these conditions can even have a greater impact on health than medical care alone.
That's why we seek to better understand your everyday life and help connect those in need to resources and services.
